Report on the First Phase of the project titled “Pilot Program for Prevention of Cervical Cancer by HPV Vaccination in Bangladesh”

The above mentioned project started it’s activities in March 2008 on receipt of a sponsorship letter from Sayed Yamin Bakht of Grameen Phone Ltd Bangladesh. For implementation of HPV vaccination in Bangladesh a committee named “Bangladesh Cervical Cancer Vaccination Program Committee (BCCVPC) was formed in March 2008.

 

Since then the members of BCCVPC together with Oncology Club of Bangladesh are working hard. The committee is chaired by the chairman of the Department of Obstetrics and Gynaecology of Bangabandhu Sheikh Mujib Medical University (BSMMU) by virtue of his post. While member secretary of the committee is the professor of Gynaecological oncology of the department. Prof. ABMF Karim, Chairman Oncology Club is the advisor of the committee.

                       

 

Within a short period project concept paper was prepared by the first week of April and sent it to all the concerned people. The committee decided to give CERVARIX to 50 unmarried girls of Bangladesh aged 9-13 years. Members had to work hard for the registration of the vaccine in Bangladesh. A letter was written to the Managing Director of GlaxoSmithKline of Bangladesh Ltd. to make the vaccine available here. Also a letter was written to the Director             of the Directorate of Drug Administration, Bangladesh for permission of importing 150 doses of CERVARIX for use. We made personal communication with above mentioned officials several times over telephone and other ways. Ultimately the vaccine was registered by the Govt. of Bangladesh on 9th September 2008. But the vaccine became available for use in December 2008.

We informed the Honorable Adviser, Ministry of Health and Family Welfare, the Secretary of the same Ministry and the Vice    Chancellor of Bangabandhu Sheikh Mujib Medical University for their kind permission to start the program in the Department of Obstetrics and Gynaecology of BSMMU. Our honorable Vice Chancellor issued a letter giving his permission on 22.06.08 to start the program in the department.

Then we applied to the chairman of the Departmental Technical Committee of the Department of Obstetrics and Gynaecology of BSMMU for their permission. A permission letter was issued on 30.08.08 .Next application in structured form for ethical clearance of the Ethical Review Committee of BSMMU and Bangladesh Medical .               

           

Research Council (BMRC) was submitted. Members of the ethical review committee of BSMMU raised several issues to be solved before using the vaccine in Bangladesh and without scientific background they denied to give clearance to start the program. We have to clarify their questions several times and had to submit the summary and abstract of the project for 5-6 times. Ultimately clearance from the BSMMU committee was obtained on 02.12.08 and clearance from that of BMRC was obtained on 23.12.08.

Without ethical clearance from BMRC we failed to obtain the fund from Grameen Phone Ltd. After getting the clearance and submitting it to Grameen Phone Ltd. the fund could be released.

A launching Program which was sponsored by GlaxoSmithKline Bangladesh Ltd. was arranged at the ballroom of Hotel Sheraton on 27th   December 2008.

Honorable Health Adviser Dr. AMM Shawkat Ali, Ministry of Health and Family Welfare Peoples Republic of Bangladesh, attended the program as the chief guest, Mr.AMM Nasiruddin, Secretary, Ministry of Health and Family Welfare Peoples Republic of Bangladesh, Prof Md. Nazrul Islam, Vice Chancellor of BSMMU, Prof Dr. M Faiz Director General of Health Services of Bangladesh       and representative from Grameen Phone Ltd were the special             guests of the program. Prof. ABMF Karim, adviser of BCCVPC chaired the program.

                                                                                                                                                                       

                       

                       

                       

In the program 15 of the preselected 50 girls were vaccinated by  the 1st dose of Cervarix.

                                   

 

                                   

                                   

 

 

                                   

An elaborate consent form regarding HPV vaccine was signed by the participant and their guardians before giving the vaccine. A vaccination card was duly filled up and half of which given to the vaccinee and the other was kept for our record. Tk-500/- paid to each girls as conveyance and fooding. Rest 35 girls were vaccinated on 28.12.08 at the department of OBGYN of BSMMU.      They were followed for any side effects and complications. The girls were provided one strip of tablet Paracetamol (500mg) for their use if any reaction occurred. Only 2-3 girls reported mild fever.

                       

Second dose of vaccine was given on 28th January 2009 at the department of OBGYN of BSMMU. Only one girl had fever 102o F and she was vaccinated after 3 days. Up till now no serious adverse reaction was reported.

3rd dose of the vaccine was given on 28th June 2009 at the same venue. Two girls failed to come for the 3rd dose of vaccine. Blood was collected form each of the girls before giving he 1st dose of the vaccine and one month after the 3rd dose. Blood will be tested for antibody against HPV to see the level of immunogenicity of the HPV vaccine.

The cost of each vaccine has been decided as Tk-3607.91/= (US $- 53.05) by negotiation between GlaxoSmithKline Bangladesh Ltd and the members of BCCVPC.
